Lyoplast Mix-MAX: Differences in Mineralized and Demineralized Forms

The mineralized powder of spongy and compact bone is a basic material, it is used in all types of bone grafting. The material has a conductive potential and fills the required defect volume. For large volumes of intervention, it is advisable to use a mixture of spongy and compact materials 1: 1. If the volume of bone added is in the area of ​​a single tooth or implant, a spongy material is more suitable. Because its particles are relatively smaller and in total volume the surface area will be larger, and there will be no discharge zones. With a large volume of bone grafting, our task is to ensure that between large particles of compact bone, which look like small fragments, are located particles of mineralized spongy powder and the surface area is as large as possible.
 
Spongy demineralized powder is used to replace only the included defects. These can be: cysts, holes of extracted teeth, resection of the apex of the tooth root. The demineralized material does not have a mineral skeleton and will dissolve relatively quickly, so if we use only it, we will get a sufficiently large shrinkage of the graft volume in height.
 
Demineralized compact bone powder has an induction potential and is used only as an additional component. All substances that are contained in the intercellular substance of the cancellous bone when it is compacted by compacting the layers repeatedly increase their concentration. Demineralized compact bone powder contains several times more different classes of bone morphogenetic proteins than spongy bone. One of the mechanisms implemented by these compounds is the following: they cause the perivascular cells to migrate from the surface of the carotid sinuses. They are located much where and will be closest to the tooth-jaw area in the region of the carotid arteries. Adventive or perivascular cells, through the generation of mesenchymal cells, can then be transformed into cells of the walls of blood vessels and into cells of the bone line. The same cells form bone cells or blood vessels with a good environment. As a result, the bone bio-implant grows into vessels in its volume much better and faster. For this demineralized powder must be in the entire volume of the material. The recommendation is relevant in case of directed tissue regeneration, in sinus-lifting operations, in patients with periodontitis, in osteosclerosis or bone grafting in 1 type of bone.
