A clinical evaluation of the effectiveness of the use of demineralized spongiosa "Lyoplast®" in the treatment of various forms of alveolitis and tooth-containing cysts in pediatric and adult dentistry has been carried out.
In the treatment of patients using the new technology using Lioplast®, inflammatory reactions were quickly stopped, the bone tissue was restored in full, the full-fledged permanent teeth erupted in children into the correct position in the dentition.
A positive result was observed in 100% of cases, in contrast to the preparations Alvozhil, Kollapan-L®, Li-Tar®, Kolapan-L®, which gave various complications.
Aim of the work - To conduct a clinical assessment of the effectiveness of using Lipelast® demineralized spongiosa in the treatment of various pathological processes in adult and pediatric dentistry.
The first group consisted of 120 people (82 women, 38 men) aged 20 to 65 years with various forms of alveolitis.
All patients were divided into 3 subgroups: CG-1 included 25% of patients. They have a hole of the extracted tooth filled with the drug "Alvozhil®".
KG-2 accounted for 27.5% of patients. In this group, the drug “Collapan-L®” was used.
The main group (FG) included 47.5% of patients. The treatment was carried out using a new method using osteoplastic material “Lioplast®”, saturated with low-frequency ultrasound using the combined antimicrobial agent butol (a combination of streptomycin and genosol). All children underwent cystectomy.
Patients were divided into several groups: in 43 the germ of a permanent tooth was removed, in 127 the germ was retained; of these, osteoplasty was performed in 65 patients using 3 types of osteoplastic material: Li-Tar®, Collapan®, Lioplast®.
All patients underwent visual examination and radiological control.
A statistical processing of quantitative data.
Results. During clinical studies, it was revealed that in patients with alveolitis in KG-1 and KG-2, the bone tissue did not fully recover, atrophy of the alveolar process was noted in the area of the extracted teeth.
This is explained by the fact that Alvozhil® is not an osteoconductive drug, its action is reduced only to the removal of pain and inflammation.
“Collapen-L®” activates the mechanism of regeneration of the affected bone tissue, however, it does not stop inflammation, which minimizes its osteoplastic properties.
Inflammatory reactions of the tooth cavity were stopped more quickly in patients in the EXHIBITION, since demineralized spongiosis "Lioplast®" along with osteoconductive properties, has a pronounced prolonged antimicrobial action.
After transplantation of this material, the first signs of bone regeneration appeared at the end of the first month.
Bone tissue was completely restored in 2.5-3.5 months after osteoplasty.
Long-term results of treatment of children with tooth-bearing cysts without the use of osteoplasty showed that in 36% of cases, the formation of a residual defect of the alveolar process, a decrease in its height, retention and delay of teething are observed.
In patients with plastic surgery with synthetic hydroxyapatite in combination with xenogenic collagen Li-Tar® and Kolapan-L®, in 17% and 100% of cases, respectively, observed complications of inflammatory nature, sequestration of the rudiments, retention of teeth, irregular position of the permanent tooth during eruption. in the dentition, obliteration of the channels, the pathology of enamel.
A new method of treating tooth-containing cysts in children has been used in 34 patients.
After removal of the cyst, the germ of the permanent tooth was left in the bone cavity, filled with demineralized Lioplast® spongy allocuse, followed by reposition of the germ.
Pre-bone cavity and alloimplant were treated with a broad-spectrum antibiotic solution using low-frequency ultrasound.
In such children in 100% of cases received a positive result.
All patients had eruption of full-fledged permanent teeth in the correct position in the dentition, the bone tissue recovered within 2-4 months; in the early and late postoperative periods, inflammatory complications were not detected.
Thus, clinical studies have shown a high efficacy of the use of allogeneic spongiosa "Lyoplast®". in adult and pediatric dentistry in patients with alveolitis and tooth-bearing cysts.
Overview of the properties and criteria for the selection of materials in different clinical situations: autografts, alloimplants, xenoimplants, hydroxyapatite, synthetics, glass ceramics, composites, etc.
